FOXO4-DRI is a senolytic peptide designed to target cellular senescence, a condition in which cells permanently stop dividing and enter irreversible growth arrest. These senescent cells accumulate in tissues and release a variety of pro-inflammatory cytokines, chemokines, and proteases, collectively referred to as the Senescence-Associated Secretory Phenotype (SASP). 

SASP is strongly linked to chronic inflammation and has been associated with multiple age-related conditions, including cardiovascular disease, neurodegenerative disorders, and physical frailty.

The mechanism of FOXO4-DRI is highly selective. In senescent cells, FOXO4 accumulates in the nucleus and binds to the tumour suppressor protein p53. This interaction prevents p53 from triggering apoptosis.

As a D-retro-inverso (DRI) peptide, FOXO4-DRI is structurally modified to resist proteolytic degradation, thus ensuring stability and persistence in biological systems. It functions by competitively binding to p53 at the same interaction site used by FOXO4, thereby disrupting the FOXO4–p53 complex. Once this bond is broken, p53 is reactivated and able to initiate the apoptotic pathway, leading to the selective removal of senescent cells.

In preclinical animal studies involving aged and progeroid models, FOXO4-DRI administration has been associated with:
